The only way that can happen is if Outlook is not sending USER/PASS
commands.  Most servers don't support them anymore as they are not secure at
all.  Maybe Outlook finally dropped support for them as well?  Who knows.

There is no way to diagnose this problem further without seeing a log of the
actual command/response packets that are sent back and forth.  You can get
that by using a packet sniffer, such as Ethereal/Wireshark.
