Look at Section 5.2.2

Yes. Creating such a message is just a matter of splitting your source
message into multiple pieces and then placing each piece into its own
message wrapper for sending. Decoding such a message is no different than
decoding any other MIME-encoded message, other than it is your own
responsibility to reconstruct the original message after you have all of the
fragmented pieces available.
RFC 2046, Section 184.108.40.206 explains how to do all of this.
