That is because you are using it wrong.

DO NOT connect to a DNS server manually. TIdSMTPRelay does that
internally for you. All you have to do is set the DNSServer property.

DO NOT call SendMsg() directly. You need to call Send() instead. All
SendMsg() does is dumps the raw TIdMessage data into the socket and
nothing else. Send() is what does all of the work of figuring out the
remote servers, connecting to them, and issuing commands to them.
